Next Step (Rehabilitation Center)

What is Next Step at Felician Village?

Next Step offers transitional and rehabilitation therapy services. Next Step is a 20-bed transitional care household dedicated to the needs of its patients. Rooms are fully furnished with cable TV and personal phones. Each room includes a full bathroom with private shower, providing convenience to every patient. Next Step is a household separate from our long-term care household, focusing on short-term comprehensive rehabilitation and meeting your rehab goals.

What health conditions can Next Step Rehab help me recover from?

Felician Village has been offering rehabilitation services for over 30 years. While here, patients have recovered from such conditions as hip or knee replacement, back surgery, stroke, wound care, IV therapy, and complications related to medical illness such as congestive heart failure and/or pneumonia.

Can I reserve a room prior to my stay?

Yes, you can pre-register by calling 920-684-7171 ext. 231. This will make your discharge from the hospital easier and faster. If Next Step is not full, there is no cost to reserving your room, although due to the demand for our rehabilitation services and specialized household, Next Step may be filled at times.

How do I pay for my rehab stay at Next Step?

Next Step accepts all forms of payment; Medicare A and B, private pay, private insurances, Medicare Advantage policies and Medicaid to cover expenses.

What paperwork needs to be filled out before I can be admitted to Next Step?

All patients must have a history and physical completed by their doctor, with updated medical information including their medications. A clinical assessment is completed prior to admission by our Rehab Clinical Manager. Demographic information is filled out by patient or responsible party prior to admission.

What do I need to bring with me for my rehab stay at Next Step?

We recommend 7-10 days of comfortable clothing. You will be in therapy most days and clothing that supports your therapy needs are best. For some patients, this may mean larger pants or tops for ease of dressing while in rehab. We will take care of your laundry needs if you choose while you stay at Next Step. We will provide you with your personal care items. However, if you have specific needs related to these items, you can bring the type you like. We encourage patients to bring items that help them feel comfortable and assist them in being away from home. Don’t worry if you forget something, you can easily pick it up at the Village Market located in the Village Square.

What is included with my stay?

Next Step offers physical, occupational and speech-language pathology seven days a week. A country kitchen is the center of the rehab household and is a common meeting place for friends and family. Three meals are prepared for you every day and our registered dietician is available to meet with you when needed. The quality of life coordinator arranges daily activities that are resident-centered and focused on mind, body and spirit. Felician Village has a resident-first philosophy that places the patient at the center of all care.

The household has two family living areas that provide residents, family and friends access to board games, Internet, the Wii and movies/DVDs to watch at their leisure or an opportunity to relax by the fireplace. The Village Square is connected to Next Step and offers patient amenities such as the HFM Clinic, Village Café, Blessed Angela Chapel, Premier Hair Studio at Felician Village and massage therapy, and much more. All of these services are accessible to our patients, their families and friends.

The Gardens (Independent Living)

What is independent living for seniors?

Independent living for seniors refers to residence in a comfortable, easy-to-maintain, private apartment or house within a community of seniors. Any housing arrangement designed exclusively for seniors (generally those age 55+) may be classified as an independent living community. The Gardens at Felician Village is considered an independent living community for those who are 55+ and offers many amenities to keep you active!

What amenities are available?

Each apartment or garden home comes equipped with a refrigerator, stove, microwave, garbage disposal, washer and dryer. Your monthly service fee includes maintenance service on all appliances. Heat, air and water utilities are also included in the apartments. All residents have direct access to the Village Square which includes Premier Hair Studio at Felician Village and massage therapy, Blessed Angela Chapel, Business Office, HFM clinic, Next Step Rehabilitation and the Village Café and Market.

Are dining services available?

Yes. There are three dining locations available to residents. The Bistro and Wisconsin Room are available at The Gardens and the Village Café is available at the Village Square. All three of these restaurants are also available to the public along with our Felician Village catering service.

How do I pay for my meals?

A meal credit for dining services is included in your monthly fee. This amount can be used in The Bistro, Wisconsin Room and the Village Café.

I still like to cook or bake. Can I still do that?

Yes! Every apartment or garden home has a full kitchen including a full size stove, refrigerator and microwave.

I enjoy socializing with friends. What is available?

Socializing for seniors is very important to us at The Gardens. You will find many common areas available for socializing throughout our campus. One in particular is our fabulous Glab Lounge, which houses our in-home movie theater system, bar/socializing area, Wii bowling, bingo, and so much more! There are also areas available for residents to reserve for family gatherings. Our three restaurants are also wonderful places for socializing with family and friends.

Am I required to pay a membership or any other type of fee prior to moving in?

In the past, residents were required to pay a membership fee but that changed in 2000. You are not required to pay a membership fee. However, a security deposit equivalent of one month’s rent is collected at the time of contract signing.

Is medical care provided?

Medical services are not provided at The Gardens. Our campus offers the convenience of an on-site HFM Clinic in the Village Square. Residents can contract with an outside homecare agency while living at The Gardens.

Will my insurance cover any of my accommodation fee?

Most insurances will not cover the cost of your accommodation fee at The Gardens.

I am still driving. Is parking available?

Yes. If you choose to live in a garden home an attached garage is included with the home. An underground heated parking garage is available for an additional fee to those living in the apartments.

Are religious services provided on campus?

Yes. Catholic Mass and Lutheran/Protestant services are offered in the Blessed Angela Chapel located in the Village Square. There is also a Pastoral Care team to help with your individual spiritual needs. Rosary is offered nightly in The Gardens reflection area. We welcome all faiths.

My family from out of town comes to visit and stay with me. Can they stay at The Gardens?

Yes. They can either stay with you in your apartment or garden home or in one of the fully furnished guest suites available at The Gardens. If they choose not to stay on campus, there are local hotels in the area.

Is smoking allowed?

Smoking is not allowed anywhere on the Felician Village campus.

Are pets allowed?

Pets are allowed to live in our garden homes only. They are allowed for visits in the apartments, but not overnight.

The Court and The Villa (Assisted Living)

What is assisted living?

Assisted living is a residential living option for individuals who typically can no longer live independently. These state-regulated and monitored facilities provide or coordinate services to meet your individualized needs in ways that promote your independence and reflect your personal life. Assisted living facilities also coordinate services to meet your unscheduled needs as they arise.

What assisted living options are available at Felician Village?

  1. The Court, a Residential Care Apartment Complex (RCAC), is a 48-apartment building with 24-hour care. Residents have their own apartment but need help with some activities of daily living or may need medication management/administration.
  2. The Villa, a Community Based Residential Facility (CBRF), also has 24-hour care and includes 32 rooms, 16 of which are in a secure household for residents with early- to mid-stage dementia. The other 16 rooms are for those with multiple medical conditions who generally need more help with activities of daily living.

How will I know which option I may need?

After you make the decision to live at Felician Village, the assisted living registered nurse will complete an assessment to determine which facility best meets your needs. During this assessment, we discuss what services you and your family would like provided. This information is then used to develop your individualized service plan.

How will my family be involved?

At The Court and The Villa, family support is highly encouraged. Families can visit when it is convenient for them and you. You and your family can be reassured that our caregivers will call your family if we see a sudden change in your condition or notice something out of the ordinary. Family members can accompany you to physician appointments, take you out shopping or for entertainment.

St. Mary's Home (Nursing Home/Skilled Nursing Facility)

How do I participate in planning my care?

You will be invited to the care plan meeting when it is scheduled. If you have any concerns or questions, please ask a staff member.

Am I able to leave the premises?

It is important that St. Mary’s staff knows the location of all residents. If you intend to leave the premises, please inform the nurse and sign out at the front desk.

How much clothing and what kind should I bring?

Bring clean, adequate clothing as appropriate to your activities, preferences and comfort. Seven to ten days change of clothing is recommended.

Is there a telephone in my room?

No, you must arrange for your primary phone service and bring a telephone. Cell phones can be used in St. Mary’s.

Is there a television in my room?

No, you must provide your own television. There is no charge for St. Mary’s local service. If you want additional cable TV channels, you must contact Comcast cable for installation service. There is a small charge with coupon (can be obtained from Social Services) for installation; there will also be a monthly cable charge from Comcast cable. All electrical equipment (TV, DVD, radio, etc.) is to be checked by the Maintenance Department for electrical safety prior to use at St. Mary’s. No extension cords or power strips can be used in residents’ rooms.

How do I get my laundry done?

If your family does your laundry, they should provide a laundry bag or basket. It is recommended that clothing be labeled with your full name. If St. Mary’s does your laundry, your clothing will be labeled with your full name.

Is there a lost and found?

For laundry, unmarked clothing is located in the Social Services wing – Room 105. If clothing is not claimed after 30 days, it is donated to charity. For other personal items, lost and found is at the front desk. An online catalog is available to staff.

How will I receive my medications?

You will receive the medications through the pharmacy provided by St. Mary’s.

Can I bring my electric scooter?

All mobility devices (cane, walker, wheelchair and electric scooter) will be labeled “property of: resident name." The therapy and maintenance departments will evaluate the device(s) for safety and function.

When can visitors come?

Visitors are welcome at any time. The reception front door is on an automatic timer seven days a week. The door automatically opens at 6:30 a.m. and closes at 9 p.m. every day. To access the facility after hours, enter the parking lot on 21st Street and proceed to the lighted employee entrance. You must ring the button located to the right of the inside sliding door. The Next Step staff will answer the call.

Can I have pets?

If your visitor has a pet, it must be on a leash or in a pet carrier. No birds are allowed.

Can family/friends join me for a meal?

We welcome your family/friends to join you for a meal at any time. Arrangements are made and guest meals are paid for at the reception desk.

Can I have a party?

Are balloons allowed?

St. Mary’s is a latex-free environment so only Mylar balloons can be brought into the facility.

What activities are included?

A monthly calendar of activities is provided to all residents. Typical activities include exercises, poker bingo, sing-alongs, ring toss, bingo, current events and more. Activities provided include:

  • Music programs: sing-alongs, rhythm band, name that tune and music appreciation.
  • Mind matters: IN2L system, quiz times, trivia, reminisce, story times and the daily newspaper gets read every day to our residents.
  • Sensory stimulation groups to touch on all senses: discussion groups to current holidays, seasons with enhancing all senses during these groups.
  • Active recreation: ball toss, bean bags, target toss, Wii bowling, stretch exercises done 4-5 times a week.
  • Variety of daily table games: Bunco, poker bingo, cover all dice, skunk, Bingo, word games, Wheel of Fortune, dominos, Connect Four, Sorry, Yahtzee, card games, Sheepshead, Bridge and poker.
  • Monthly acts to gain socialization from other parts of the campus are held a couple of times a month to include live entertainment, ice cream socials, slide presentations, dance groups and birthday socials. There is a monthly resident council meeting.

Can I get my hair done?

Premier Hair Studio at Felician Village is located in the Village Square. All typical salon services are available at a reduced rate for all St. Mary’s residents. Appointments should be made by calling ext. 311.

Where can I buy cards, gifts, convenience items, bakery, etc.?

The Village Market is located in the Village Square next to the Village Café.

The hours for the Village Market are 7:30 a.m.-3 p.m. Monday through Friday and 10 a.m.-2 p.m. weekends and holidays.

Where can I get lunch, a snack or just a cup of coffee?

The Village Café is located in the Village Square. The hours are 7:30 a.m.–3 p.m. Monday through Friday and 10 a.m.–2 p.m. weekends and holidays.

Can I attend church services?

Everyone is welcome to attend all religious services. There are a variety of services offered for several denominations. Presently, we have the opportunity to offer daily Mass, which is held at 9:30 a.m. There is also Mass offered on Saturdays at 4 p.m.

We also offer Lutheran services on the first, third and fifth Wednesday of each month at 10:30 a.m. Additionally, there are other Protestant services on the second and fourth Wednesdays at 2 p.m. All services are held in the Blessed Angela Chapel located in the Village Square.

For those unable to be physically present in the chapel, they may participate in the privacy of their rooms, as all services are available on television. Please use channel 12.1.
